The Utah Uniform Interstate Depositions

and Discovery Act

Under The Utah Uniform Interstate Depositions and Discovery Act the following items are necessary in order to Domesticate a Foreign Subpoena:

The court requires original documents with the wet signature.
Please mail to: 2637 N. Washington Blvd. #336 North Ogden, UT 84414.
Utah Process Service need 3 copies of the following:
1) SUBPOENA (to be issued - Utah court)
2) APPLICATION FOR SUBPOENA UNDER THE UTAH UNIFORM INTERSTATE DEPOSITIONS AND DISCOVERY ACT
3) UTAH DISTRICT COURT COVER SHEET
4) SUBPOENA (Foreign Subpoena to be domesticated), and any other documents you want served.

The fee for filing the Foreign Subpoena will be determined by the Service Area of the court. The court also requires a $35.00 FILING FEE (please include this check made payable to the court).
